HIPAA Notice

1. Dayhawk's role

In providing teleradiology interpretation services, Dayhawk Telemammography acts as a Business Associate to the healthcare organizations (Covered Entities) we serve, as those terms are defined by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We execute a Business Associate Agreement (BAA) with every partner organization prior to go-live.

2. Safeguards

Our workflows, infrastructure, and reading environments are designed around HIPAA's administrative, physical, and technical safeguard requirements, including access controls, encryption of data in transit, workforce training, and auditability of access to protected health information (PHI).

3. No PHI through this website

This public website is informational and is not a channel for PHI. Do not submit PHI through the contact form or by unsecured email. Clinical data exchange with partner organizations occurs exclusively through secured, contracted interfaces established during onboarding.

4. Patient rights

Patients' HIPAA rights — including access to records, amendments, and accountings of disclosures — are administered by the Covered Entity that provided care. If you are a patient, please contact the imaging facility or health system where your study was performed. As a Business Associate, Dayhawk supports these requests through the Covered Entity as required by our BAAs.

5. Breach notification

In the event of a breach of unsecured PHI, Dayhawk will notify affected Covered Entities without unreasonable delay and in accordance with the HIPAA Breach Notification Rule and applicable BAA terms.
